Team Member Forms
While most of the forms that current Team Members need are in the Team Member and HR Portals, there are a few forms that our former Team Mates may need access to as well. These are found below. If you have any questions, please contact Human Resources at

Team Member Education
MWAS has our own on-site educator who teaches our CPR classes. We have our own equipment and are an American Heart Association CPR site. All our CPR classes are offered free to team members.
In addition to CPR, MWAS provides other skills and enrichment classes throughout the year. These classes are also provided free to team members.
MWAS also offers tuition reimbursement for EMT, EMD, and Paramedic classes. See Human Resources for details.
Team Member Uniforms
MWAS provides uniforms to our team members. Full time team members are issued two full uniforms including 5.11 Tactical boots and weather resistant coat. PRN team members are issued one full uniform, also including 5.11 Tactical boots and weather resistant coat. New uniforms are issued on an annual basis.
Full-time team members have the choice between two medical coverage plans through Anthem. These plans offer low deductibles and choice of physicians to meet financial and healthcare needs. MWAS covers 50% of the monthly premiums. Coverage for spouse and family are also offered.
Dental & Vision Insurance
For information on our vision coverage options, Full-time team members can opt for dental and vision coverage. We offer a choice of two dental plans. Our vision coverage allows for frames every 12 months. Both dental and vision plans are managed through The Principal.
All full-time team members are eligible to participate in our group life insurance plans through MetLife. Coverage amounts start at $10,000 and increase to $240,000. Premiums are very competitive for the benefit of our team members.
We offer a selection of supplemental insurance coverage through Aflac. Our current coverage options include Accident, Cancer, Critical Illness, and Short-term Disability policies.
Safeharbor 401(k) Savings Program
Our retirement savings program is offered to both full- and part-time team members who have been with the company for at least 6 months. This Safeharbor 401(k) is managed through American Funds. MWAS matches team member contributions up to 4%.
Verizon Wireless Discount
All MWAS team members may enjoy a 19% discount on phones and services through Verizon Wireless.
